How Goodhealth Policy is renewed

For this policy, you should have valid citibank credit card and premium will be debited in citibank credit card.

We will send Renewal notice to you 50 days before your policy expires.

We attach the following for this.

1) Renewal Notice.  2) Application form for any modification in the renewal 3) Premium rates.

Goodhealth policy is renewed in the following basis.

1) Automatic
2) Optional.

If you want to renew the policy without any intervention from you (you need not submit any form for this), the policy
will be renewed automatically for the persons you have covered last year.

If you want to change plan options, add names or delete names of the persons covered last year, then you have to intervene
in the renewal and you have to submit application form sent by us along with renewal notice.  Or you may download the same
and fill it up and send the same to

This form should be sent to them 15 days before your policy expiry. (otherwise your earlier policy will be renewed automatically).

If you don't want to renew the policy, you may send the communication to the above mentioned address or you may fill up the following information in the application form sent by us and just tick the box "(A)  I DO NOT WISH TO RENEW" (found in page 3).

1) Your name, 2)card number, 3) earlier policy certificate number 

This form should be sent to them 15 days before your policy expiry. (otherwise your earlier policy will be renewed automatically).

Examples to have modification in the renewal:

Assume A, B,C,D,E,F were covered in the Mediclaim Policy and due for renewal. (Plan option is 6 for all)
'A' is Card Holder.

Case: 1 PLAN CHANGE: (to change plan 6 to 12 for 'F')

'A' has to fill up the application form for renewal by writing all the names (A to F) with plan options, date of birth, relationship etc. and for 'F',  revise plan option should be mentioned. (In this case, A to F  will be renewed for the plan options mentioned  in the form)

Case: 2 Deletion of member ( to delete member 'D')

'A' has to fill up the application form for renewal by writing all the names (A to F) with plan options, date of birth, relationship etc. without writing the name 'D'. (In this case, all the persons mentioned in the form will be renewed for the plan options mentioned in the form and 'D' will not be renewed.)

Case: 3 Additon of member ( to add member 'G')

'A' has to fill up the application form for renewal by writing all the names (A to F) with plan options, date of birth, relationship etc. and also  writing the name 'G' along with date of birth, plan options, relationship with the card member.. (In this case, all the persons mentioned in the form will be renewed for the plan options mentioned in the form and 'G' also will be insured along with others mentioned in the form)

ID cards will not be issued in case of renewal.  Earlier Goodhealth Policy ID card is valid for this renewal also.

Duirng the Policy period, all address changes should be intimated to our office and we will update the changes in our database so that next year renewal  notice will be sent to the new address.

CLAIMS

For medical expenses claim, first ascertain that the person is covered under our mediclaim policy cover section.

Then,  please contact TTK Healthcare TPA  Pvt.Ltd, Chennai (Toll Free : 1800-425-7575 or 1800-425-8885 or cell phone users may contact 98452 48885) and provide policy certificate number well in advance i.e. 2 days prior to date of hospitalisation (or hospitalization is done on an emergency basis) and ask for pre-authorization request. (They will guide you). 

They will process your request (according terms and conditions of the policy) and found eligible, they will issue Pre-Authorizaton approval letter to the hospital directly (if the hospital is figured in TTK Healthcare TPA  Pvt.Ltd network list)
and settle the eligible amount to the hospital.  Balance amount if any, should be borne by you.

Or, for any reason, the pre-authorization request is denied or the hospital is not in the network hospitals of TTK Healthcare TPA  Pvt.Ltd , then you settle the hospital bills and claim with TTK Healthcare TPA Pvt.Ltd.  You can download a claim from www.ttkhealthcareservices.com or you may contact them over phone and request for claim form. They will guide you how to proceed in the claim.

The settled amount will be deducted from the total sum insured of the person for whom the claim is settled and balance sum insured will become sum insured for future claims if any in that policy period. If there is no sum insured available under the
mediclaim policy section for that person, then any subsequent claims will not be entertained for that person during the policy period.

E.g

Person X is insured for Rs.2 lacs under Mediclaim Section. and the policy period is 01.10.2006 to 30.09.2007.

If X is admitted in the hospital and claim is settled for Rs.75,000/-, then balance Rs.1,25,000 is only available for future claim during 01.10.2006 to 30.09.2007 (Rs. 2 lacs - Rs.75,000/-)

If X is admittted in the hospital and another claim is settled for Rs. 85,000/- then he will have balance of Rs.40,000/- only for future claim during 01.10.2006 to 30.09.2007 (Rs.1,25,000 - Rs. 85,000/-)

If X is again admitted and the cost of medical expenses exceeds Rs,.40,000/-, then this Rs.40,000/- only will be considered for settlement and no more claim will be entertained for Mr.X. in that year.(Rs.40,000/ - Rs.40,000/-)

If the policy is renewed from 01.10.2007 to 30.09.2008, then Mr.X is eligible to claim under the renewed policy (hospitalisation is on or after 01.10.2007) and limit of sum insured is the sum insured mentioned in the policy.  If he claims again, and the amount settled will be deducted in sum insured and balance sum insured is available for future claims as stated above.

If X is taken for Personal Accident cover only and not opted for Mediclaim Cover, then any medical expenses incurred by Mr.X during hospitalisation cannot be paid.

In Personal Accident Policy, if Mr.X dies in accident, then the sum insured mentioned in the policy in personal accident section will be paid to the nominee registered with us (in Mr.X's nomination form) after obtaining required legal documents.

CANCELLATION:

To cancel this Goodhealth Policy written request should be sent to indiaservice@citicorp.com or to CITIBANK CARD CENTRE, (Attn.Goodhealth Policy, No.2 Club House Road, CHENNAI-600 002. or you can email to us nia25@vsnl.com

If the request for cancellation received by citibank/us after commencement of policy, then refund percentage will be applied as follows:

Request received between 0-60 days from the date of commencement of policy, 100% premium will be refunded.

Request received between 61-90 days from the date of commencement of policy, 50% of the premium will be refunded per person

Request received between 91-180 days from the date of commencement of policy, 25% of the premium will be refunded per person .

Request received after 180 days from the date of commencement of policy, no premium will be refunded.

Cancellation can be done in following manner also:

1) By choosing person  (In this case, both Personal Accident policy and Mediclaim Policy for that person can be cancelled.)

2) By choosing person,  insurance coverage. (In this case, Personal Accident policy or Mediclaim Policy Section can be
    cancelled for that person)

3) Whole policy certificate  (In this case, all the persons mentioned in the policy certificate will be cancelled and they will not
    have insurance cover in Personal Accident Policy and in Mediclaim Policy.)
