Request for Policy Certificate

I  have not received my Policy Certificate.  When will I get it ?

Policy certificates are normally despatched by courier within 30 days from the date of commencement of  policy.  In  case you have not received your policy, contact us nia25@vsnl.com  and advise us your correct address so that we can reach your policy to you. or write a letter to the above address. 

I  have misplaced my policy and I want a duplicate.

Please mail to us your request for your policy certificate indicating  your name, card number, Citibank order 
number,  your last year's policy certificate number (in case of renewal only) and your address.  We shall arrange 
to  reach your  policy to you.

Renewal of  Policy Certificate

want to renew my policy.  What shall I do ?

Cancellation  of  Policy Certificate
      
I want to cancel my good health policy. What shall I do ?

In case  renewal of policy was effected against  your option for non-renewal, the said policy may be cancelled from its inception, at your request  to either Citibank/the Insurance Company, within the prescribed time limit, and full refund of premium shall be made over to Citibank on your behalf  by the Insurance Company provided no claim has occurred upto the date of cancellation.   Citibank's confirmation of  receipt of request from you for such cancellation will be binding upon you.

In all other cases you may at any time cancel this policy and in such event the Insurance Company shall allow refund of premium at Company's short period rates only as indicated below, provided no claim has occurred upto the date of cancellation.

Can I claim Income Tax deduction in respect of a policy, which has been cancelled at my request ?
    
In the event of the insured  requesting for cancellation of this policy and seeks refund of premium, any certificate issued to the insured for the purpose of claiming deduction under Section 80-D of the Income-Tax Act, 1961, shall also be deemed to be cancelled and the insured cannot claim any deduction for Income-Tax purposes, against the such policy or certificate.

Miscellaneous matters

The amount mentioned in the Premium Certificate for Income Tax purposes does not tally with the premium paid by me.

The premium certificate issued to you is not a receipt for premium paid. It is issued to enable you to claim income 
tax benefit .

As per Section 80-D of the Income Tax Act, you are entitled for claiming Income Tax Rebate  in a financial year in respect of premium paid by you for mediclaim insurance for covering self, spouse, dependent children and dependent parents only.

Therefore the  amount mentioned in the premium certificate need not necessarily tally with the premium paid by  you.

Am I not entitled to family discount and free health check up which are normally available in any individual mediclaim policy?

This  being a group policy, there is no provision for family discount  and free health check up.   
However, being a good health policy holder you have the benefits of lower rates of premium (except in mediclaim policy for those crossed 60 years.)

I am not a card member.  Am I eligible for this Good Health Insurance Scheme ?

Only valid Citibank Card Members are eligible for this Good Health scheme.

I intend  to close my Citibank credit card.  Will I continue to be covered under the Good Health policy ?

You will be covered under your present Good Health policy till its expiry and the policy will not be renewed next year.

Can I cover my friends, colleagues, staff and servants under the  Good Health Insurance Scheme ?

As a Citibank card member you can cover only yourself and  / or your family members/relatives/employees  under this policy.

Is it possible for me to get my Good Health policy serviced by a New India Office nearby to my place?

Servicing of Good Health policy is centralised at this Chennai office. All your queries regarding this policy will be 
handled only by us.

Is nomination compulsory for my Good Health personal accident insurance?

Nomination facility is available for persons covered under the Good Health Personal Accident Insurance.  You 
may  nominate the person of your choice to receive the personal accident compensation on your behalf.  Nominations  would help the nominee to receive the claim monies without any delay. To avoid any legal dispute it is always advisable that the nominee may be one of the legal heirs.

Nomination form is available in the Good Health policy salient features brochure. Filled in nominations should be 
sent  to us in duplicate so that we can return one copy to you after duly registering your nomination in our records.

I require change in plan options or include new members in my existing Good Health policy.

Request for such changes can be considered only at the time of next renewal. Refer renewal procedures.

How do you determine whether a disease was a pre-existing one or not?

At the time of filling up the proposal form for mediclaim insurance under the Good Health Policy, you need to disclose  
the details of the illnesses suffered by you in  your lifetime. Failure to disclose such facts may go against you, if 
discovered  at a later date. While proposing for mediclaim insurance you must be aware of any disease for which you are/were under treatment . These matters are referred to a panel of medical practitioners to determine whether the illnesses are pre-existing or newly contracted.
